STEP ONE:
Download the Louisiana Form 16 (PDF) . Complete the form, sign and print. You will attach this form to your online enrollment confirmation or your paper enrollment form prior to submitting. 
 
STEP TWO:
Once you have completed the enrollment process online, please print TWO copies of your confirmation page from the “Congratulations” screen. Keep one copy for your records and attach the other copy to your completed Louisiana Form 16 (Blue Form) and return it to your Human Resources Department.
